FATAL CRASH INVESTIGATION
August 20, 2025

On August 16, 2025, at approximately 7:42 p.m., officers from the Lake Havasu City Police Department responded to a collision involving multiple vehicles on State Route 95 at Kiowa Blvd. Upon arrival, it was determined that a Chevrolet Equinox was involved in a rear-end collision, which forced several vehicles into each other.

Preliminary investigations determined that the Equinox was traveling south on SR 95 when, for reasons still under investigation, it failed to control its speed and collided with the rear of a vehicle stopped at the intersection. The impact of the Equinox on the second vehicle forced four other vehicles to collide with each other.

The driver and sole occupant of the Equinox, identified as 75-year-old Lake Havasu City resident Lyle Taylor, was transported to the hospital, where he was pronounced deceased. Five additional people were transported to the hospital with non-life-threatening injuries.

The cause of the crash, along with any contributing factors, remains under investigation.

Additional details will be provided as they become available.
